The Use of Service Request Form

To proceed with he genetic decoding service, Genetica needs to receive both of the following:

  • Qualified saliva sample
  • Hand-filled Service Request Form, with complete information và proper signature.

Print and fill in all the required information accurately.
Contact Genetica to retrieve the form, by one of the following means:

The mail carrier will come to collect within 2-4 working days. Genetica will proceed with your service once we receive a valid form.
Customer can track their gene decoding process on Genetica app by pressing Profiles menu > Select desired profile
